#214: Restructure GD annotation folders and update metadata

For all annotations (fall 2014 - fall 2018), restructure GD folder and file structure to match input formula for edition

Update annotation metadata with folder URLs, UUIDs, etc. for these items

Abstract and bibliography folders and files created and populated by NJR and CMS. Completed for: FA17, SP17, FA16, SP16 Annotation metadata updated.

NOTE: abstracts and bibliographies were not deleted from original annotation text files. To be completed after group discussion on 2/18/19.
